Destruction of Medical Records

HCA- HealthONE LLC d/b/a Spalding Rehabilitation L.L.C, 900 Potomac Street, Aurora, Colorado 80011, HCA-HealthONE L.L.C at Presbyterian/ St Luke, 1719 East 19^th Ave, Denver, Colorado 80218 and HCA-HealthONE LLC d/b/a Spalding Rehabilitation L.L.C at Rose Medical Center, 4567 E 9^th Ave, Denver, Colorado 80220 will destroy inactive medical records for treatments delivered prior to 12/31/1997 for persons 28 years or older as of 12/31-1997 on June 30, 2009 . The following names have been used for the facilities now listed as HCA-HealthONE LLC d/b/a Spalding Rehabilitation L.L.C. and will be included in the destruction:

Rocky Mountain Rehabilitation Institute, Inc.

Swedish Medical Center (Spalding Rehabilitation) – Aurora Campus (Opened 01/01/1996; Closed 9/30/1996)

Center for Spine Rehabilitation (Closed 12/31/1991)

Spalding Rehabilitation Longmont United Medical Center (Closed Inpatient 12/31/1996; Closed Outpatient 1/31/2000)

Spalding Rehabilitation at Lutheran Medical Hospital (Closed December 31, 1998)

Spalding Rehabilitation at Bear Creek Nursing Center (Opened 1/12/1999; Closed 2/28/2002)
